Psoas abscess has been classified into two types, namely the primary abscess, which presents with infection limited to the psoas muscle itself and is mostly pyogenic, and the secondary abscess, in which associated intra‑abdominal or skeletal conditions are present; Staphylococcus aureus is the most common causative organism in primary cases, while secondary cases are mostly associated with adjacent tissue infection, tuberculosis of the spine, or intra‑abdominal pathologies, with lumbar spine tuberculosis—due to its close anatomical association with the psoas muscle—being involved in 75%–83% of cases, and bilateral cases of psoas abscess being uncommon in the context of tubercular spinal disease.
